Synergy Estate is a twenty-four (24) hour seven (7) day a week Adult Residential Facility (ARF) also known as a Short-Term Residential Program. Our recovery home provides individualized evidence-based treatment by diagnosis. Together with our integrated wellness model and holistic approach this powerful combination provides a unique path to recovery for each individual client. Services provided at the ARF include, but are not limited to, room and board, 24-hour care, medication support, group and individual therapy, 12 step-based recovery groups, facilitate psychiatric inpatient admission as necessary, linkage and/or referral to most appropriate level of aftercare, and community resources.

Payment for services rendered is due and payable upon admission and prior to services rendered. The all-inclusive rate is $1,500 per day. An initial payment of $45,000 for thirty (30) days of treatment is payable in full. Payment can be made via cash, cashier’s check and/or credit card. Checks are not an acceptable form of payment.

Through this secure payment portal, you can submit your credit card payment at your convenience, 24 hours a day, 7 days a week. Before using the service, please have on hand your Synergy Estate billing letter, including your patient account number, and your Visa, MasterCard, or American Express credit card number, expiration date, and CVN (security) code.

NOTIFICATION OF PATIENT LIABILITY
The client’s length of stay at the Adult Residential Facility (ARF) will be determined by the client and his/her physician. The maximum length of stay is thirty (30) days. If the client does not complete thirty (30) days of treatment and requests to vacate the residence prior to the agreed upon treatment length, then the nonrefundable deposit of $4,500 will be retained by the ARF in addition to the $1,500 per day for service days provided. The remaining amount will be refunded within seven (7) days of the client vacating the ARF.

OPTIONS FOR PAYMENT
Insurance coverage and authorization for the level of care and treatment provided at residential facilities is based upon individual plan benefits. Synergy Estate agrees to pursue and attempt to obtain authorization and payment however there is no guarantee that any portion of the services provided will be covered under your individual plan. If payment is secured and issued to Synergy Estate then Synergy Estate agrees to issue a refund of payment in the amount issued by the insurance company up to the full amount of payment received by the individual client. In no case will a refund payment be issued to the client greater than that amount initially received by the client.
